UHS Home Care Coordinator

Coordinates and directs homecare services with physicians, nurses and hospital discharge planning staff to decide patient's appropriateness for homecare.

Regular Scheduled Weekly Hours: 24

Compensation Range: $31.15 - $46.72 per hour, depending on experience

Education/Experience

Minimum Required:

• Graduate of an accredited RN program.
• Previous experience in a certified home health agency.

License/Certification

Minimum Required:

• License and current registration to practice as a registered Professional nurse in New York State.
