Key Responsibilities

• Provide physical therapy services to patients in a home care setting
• Evaluate and assess patient conditions to develop individualized treatment plans
• Implement therapeutic interventions to improve patient mobility, strength, and function
• Document patient progress and treatment notes accurately within the EMR system
• Collaborate with interdisciplinary care teams to coordinate patient care

Required Qualifications

• Active Physical Therapist license in the state of Ohio
• Experience in a home care setting
• Proficiency with Epic EMR
• Strong clinical assessment and documentation skills

Preferred Experience / Skills

• Prior contract or travel therapy experience
• Strong communication and patient education skills
