Schedule
Shift: 2:30 PM - 10:30 PM

Assignment Details
Contract Length: 13 weeks
Guaranteed Hours: 37.5 per week

Requirements
- Active LPN license in NY
- BLS certification
- Minimum 1 year of experience in a long-term care facility
- Skills checklist matching specialty
- NURSYS report for license verification
- EMR Experience: PointClick Care

Roles & Responsibilities
- Provide direct patient care in accordance with physician orders and established care plans.
- Assist with daily living activities, including personal hygiene, grooming, and feeding.
- Monitor and document patients' health status and report changes to the nursing staff.
- Collaborate with interdisciplinary teams to enhance patient care and outcomes.
- Maintain a clean and safe environment for patients and staff.

- Floating required within the same building
