Patient Care Navigator & Case Manager
Betances Health Center
Betances Health Center is seeking a Case Manager to facilitate the coordination of patient services, ensuring continuity of care and acting as a patient advocate. The Case Manager will work closely with medical providers to help patients overcome barriers, connect with community resources, and provide ongoing support for entitlements, food pantries, durable goods, home health aides, and housing needs. Applicants should possess relevant experience in healthcare settings, conflict resolution skills, and proficiency in EMR systems.
