Contract | 13 Weeks | Days | 5×8 Hours, Monday-Friday (8:00 AM - 5:00 PM) | Onsite/Field-Based

Key Responsibilities

• Provide home visits to adult patients across the Westchester territory, with an average daily caseload of 5 to 8 patients and a weekly productivity expectation of 30 points.
• Perform a variety of case types including Start of Care, Wound Care, follow-up visits, and other adult home health services.
• Complete all clinical documentation within 24 hours of each patient appointment using Homecare Homebase (HCHB) ; RNs must also be proficient in OASIS .
• Attend periodic office meetings at the assigned care center in addition to daily patient home visits.
• Participate in weekend rotation (approximately once every 4-6 weeks) and carry on-call responsibilities during assigned weekend weeks.
• Travel up to 75 miles from the assigned care center to reach patients; mileage reimbursement is provided at the IRS standard rate for travel between patient homes.

Required Qualifications

• Active, unrestricted RN license in hand - pending licenses are not accepted.
• Minimum one full year of recent adult home health experience required; first-time travelers are not accepted.
• CPR and BLS certifications required.
• Proficiency in Homecare Homebase (HCHB) used within the past 6 months; clinicians outside this window may be considered but must complete retraining during compliance.
• OASIS proficiency required and must have been used within the past 6 months, or a retraining course must be completed during compliance.
• Reliable personal vehicle, valid driver's license, and agency-required auto liability insurance required.
• Local candidates are welcome and are accepted at the same rate as travelers.

Preferred Experience / Skills

• Prior home health and HCHB experience within the past 6 months strongly preferred.
• Comfort with driving up to 75 miles from the assigned care center and accepting all scheduled patients regardless of distance from previous appointments.
• Flexibility to float to a nearby clinic for additional patient volume during periods of lower census.
• Experience with OASIS documentation in a home health setting.
