Key Responsibilities

• Perform initial in-home pre-evaluations of patients, including assessment of health status, strengths, care needs, and preferences to guide the development of individualized long-term care service plans
• Review consumer medical documentation and/or health referral forms as relevant to each case and enter evaluation data into electronic forms for transmission
• Document concerns, conflicting information, and issues that surface during the evaluation process while emphasizing continuity of care to reduce fragmentation, duplication, and gaps in treatment
• Discuss healthcare options, supports, service vendor options, and waiver options with consumers; initiate communication between the call center and eligible consumers for enrollment into managed care health plans
• Collaborate with interdisciplinary care team members to improve care coordination, facilitate service delivery, and collect quality review data and required documentation to support outcome measurements
• Identify opportunities for health promotion and illness prevention while maintaining comprehensive knowledge of community resources, payor requirements, and network services for the target population

• Current Registered Nurse (RN) licensure in New York State
• BLS certification required prior to start date if not already obtained
• Minimum of two (2) years of experience in a home setting within the last fifteen (15) years
• Recent experience conducting comprehensive assessments of patients
• Candidates may not simultaneously conduct UAS assessment assignments elsewhere; a Conflict of Interest (COI) form must be signed prior to start
• Ability to perform hybrid telehealth and in-field assessments

• Experience working with UAS (Uniform Assessment System) criteria or similar standardized assessment tools
• Familiarity with long-term care service planning, managed care enrollment processes, or Medicaid waiver programs
• Experience working with multi-generational and diverse patient populations in community or home-based settings
• Proficiency with electronic health record systems and CRM documentation platforms
