Key Responsibilities

• Perform initial in-home pre-evaluations of patients, including assessment of health status, strengths, care needs, and preferences to guide the development of individualized long-term care service plans
• Review consumer medical documentation and/or health referral forms relevant to each case and enter evaluation data into electronic forms for transmission
• Document concerns, conflicting information, and issues that surface during the evaluation process while maintaining accurate case notes in the CRM
• Facilitate communication between the call center and eligible consumers for enrollment into managed care health plans, and discuss healthcare options, supports, and waiver options with consumers
• Collaborate with interdisciplinary care team members to enhance care coordination, reduce fragmentation, and facilitate service delivery
• Identify opportunities for health promotion and illness prevention while maintaining a comprehensive working knowledge of community resources, payor requirements, and network services

• Current RN licensure in New York State
• BLS certification required prior to start date
• Minimum of two (2) years of nursing experience in a home setting within the last fifteen (15) years
• Recent experience conducting comprehensive patient assessments
• Ability to work with technology in the workplace, including electronic evaluation and CRM systems
• Strong ability to work independently in a fast-paced, deadline-oriented environment while managing multiple complex tasks simultaneously

• Experience with UAS (Uniform Assessment System) criteria and long-term care enrollment processes
• Familiarity with managed care health plans and community-based service coordination
• Excellent organizational, interpersonal, written, and verbal communication skills with the ability to work effectively with multi-generational populations
