Speech-Language Pathologist (SLP) – PRNAscend Home Health & Hospice | Northern Utah

Ascend Home Health & Hospice is seeking a compassionate and motivated Speech-Language Pathologist (SLP) to join our growing team on a PRN basis serving patients throughout Cache and Box Elder Counties in Northern Utah.

This position offers flexibility and the opportunity to make a meaningful impact by helping patients improve communication, cognition, and swallowing abilities in the comfort of their own homes.

Responsibilities:

• Evaluate and treat patients with speech, language, cognitive, voice, and swallowing disorders
• Develop individualized treatment plans based on patient needs and goals
• Educate patients, families, and caregivers on therapy techniques and care plans
• Collaborate with physicians, nurses, therapists, and the interdisciplinary care team
• Complete timely and accurate documentation

Qualifications:

• Current Utah SLP license required
• Home health experience preferred, but not required
• Strong communication and organizational skills
• Compassionate, dependable, and patient-focused
• Valid driver's license, reliable transportation, and current auto insurance required

Position Details:

• PRN / Flexible scheduling
• Coverage area includes Cache and Box Elder Counties
• Pay per visit DOE (dependent on experience)
