Director of Social Work
Job Summary
We are seeking a compassionate and experienced Director of Social Work to join our team at the White Plains Center for Nursing in White Plains, New York. As a key member of our leadership team, you will be responsible for overseeing the social services department and ensuring the highest quality of care for our 88-bed facility's short- and long-term care residents.

Responsibilities
Oversee the social services department, including case management, discharge planning, and care planning
Work closely with the Administrator and interdisciplinary team to develop and implement care plans that meet the unique needs of each resident
Ensure compliance with state and federal regulations, including MOLST and Sigma Care system
Develop and maintain relationships with community resources and external providers
Participate in quality improvement initiatives to improve resident outcomes and satisfaction
Collaborate with nursing staff to ensure effective care delivery and communication
Requirements
Master's degree in Social Work (MSW) from an accredited institution
3+ years of experience in healthcare, with a focus on nursing home care
Strong working knowledge of discharge planning, care planning, MOLST, and Sigma Care system
Excellent communication, interpersonal, and organizational skills
Ability to work effectively in a fast-paced environment and prioritize multiple tasks
