Description

• Assess patients across both inpatient and outpatient settings and craft personalized treatment plans tailored to individual needs
• Deliver physical therapy treatments focused on enhancing mobility, strength, balance, and overall functional independence
• Work collaboratively with nurses, physicians, case managers, and a team of rehabilitation professionals to achieve the best clinical outcomes
• Educate patients and their families regarding therapy goals, discharge procedures, and home care exercises
• Diligently document evaluations, treatment notes, and patient progress in alignment with institutional and regulatory standards
• Engage in care team meetings and interdisciplinary discussions to ensure holistic and coordinated care for every patient

Education

• Doctorate in Physical Therapy (DPT) from an accredited institution or a comparable degree

Certifications

• State licensure as a Physical Therapist is essential
• Preferred certification in CPR
• Additional specialized certifications (such as geriatric or neurological physical therapy) are advantageous but not necessary

Skills

• Prior experience in a clinical setting is strongly desired
