Work Environment

You will be part of an integrated team consisting of physicians, registered nurses, physical therapists, and other healthcare professionals. This onsite role requires you to travel to various patient homes within the Lakewood area, providing you the unique opportunity to engage with patients in their own environments. Key Responsibilities

• Conduct thorough assessments to diagnose both acute and chronic conditions, utilizing clinical skills and patient histories.

• Develop, implement, and monitor individualized treatment plans, ensuring alignment with both patient needs and evidence-based practices.

• Order and interpret diagnostic tests, including lab work and imaging, to support clinical decision-making.

• Prescribe necessary medications and coordinate therapies, adjusting prescriptions in accordance with ongoing patient evaluations.

• Educate patients and their families on disease management, preventive health practices, and lifestyle choices that foster well-being.

• Maintain meticulous patient records utilizing electronic health record systems, complying with regulations and ensuring accuracy in documentation.

• Engage in quality improvement initiatives, contributing to the continuous enhancement of services and patient care protocols.

Patient Population / Client Focus

Qualifications

• Current certification as a Nurse Practitioner, with a valid and active state nursing license.

• A Bachelor's degree in nursing from an accredited program is required; a Master's degree in nursing is preferred.

• A minimum of 3 years of clinical experience in a relevant specialty is mandatory.

• Strong analytical and problem-solving skills, complemented by excellent communication and interpersonal abilities.

Compensation and Benefits

• We offer a competitive salary range of $150,000 to $180,000 per year, which is commensurate with experience and qualifications.
