EmpowerLife Health Care Services is seeking a dedicated Full-Time Nurse Practitioner (NP) to provide comprehensive community health care at our onsite Assisted Living Facility in Lakewood, New Jersey. Work Environment

This position is based in an Assisted Living Facility, offering an onsite and collaborative work environment that fosters teamwork and encourages innovative approaches to healthcare delivery. You will work within a supportive and dynamic team that values each member’s contributions and promotes a culture of continuous learning. The typical workweek consists of approximately 40 hours, with a schedule that generally runs from 9:00 a.m. to 6:00 p.m., alongside opportunities for evening clinics and participation in community outreach activities.

Key Responsibilities

• Conduct comprehensive health assessments, including physical exams and screenings, to identify health needs and develop appropriate care plans.
• Diagnose acute and chronic medical conditions, creating individualized management plans that emphasize preventive care and wellness promotion.
• Collaborate with local community organizations to design and implement health education programs, tailored to meet the diverse needs of the population.
• Provide culturally sensitive healthcare, advocating for patients’ rights and ensuring equitable access to services and resources.
• Coordinate referrals and facilitate follow-up care, ensuring seamless transitions and continuity for patients.
• Maintain accurate and compliant medical records, utilizing electronic health systems to document assessments, interventions, and outcomes clearly.
• Engage in community outreach events and health fairs to raise health awareness and offer screenings to those in need.
• Stay informed about community health trends, best practices, and relevant healthcare policies to enhance service delivery.

Patient Population / Client Focus

This role focuses on serving a diverse population, particularly vulnerable and underserved individuals and families. You will work closely with residents of the Assisted Living Facility, ensuring they receive compassionate and comprehensive care that addresses both their health needs and social determinants of health.

Qualifications

• A Master’s degree in Nursing from an accredited program is required.
• Current and active Nurse Practitioner certification, along with a valid state nursing license.
• Demonstrated commitment to social justice, health equity, and advocacy for underserved communities.
• Strong interpersonal and communication skills, allowing effective collaboration with patients from diverse backgrounds.

Preferred Experience

A minimum of 3-5 years of clinical experience, preferably in community health or public health settings, is preferred. Candidates who have a robust background in health education and community engagement will be highly regarded.

Compensation and Benefits

This position offers a competitive salary range of USD 150,000 to 180,000 annually, commensurate with experience and credentials.
