Work Environment

The role of Nurse Practitioner is situated within an onsite healthcare facility that fosters a collaborative environment, where you will be a part of a dedicated care team. This team includes a physician, other nurse practitioners or physician assistants, medical assistants, and support staff including registered nurses and care managers. Key Responsibilities

• Deliver high-quality primary care by conducting comprehensive office visits for both routine and acute health concerns.
• Administer annual wellness visits and perform detailed health risk assessments to identify patient needs and establish care plans.
• Coordinate care with multiple providers, specialists, and testing facilities, ensuring an integrated approach to patient management.
• Lead population health initiatives to enhance the health of our patient base, such as monitoring and facilitating evidence-based screenings.
• Support phone triage, provide outreach to patients, and assist in managing care transitions as necessary.
• Educate patients about their diagnoses, treatment options, and care plans to promote active participation in their health journey.
• Engage in promotional activities and community outreach for health education and awareness.
• Conduct home visits for patients who are unable to come to the clinic, ensuring every patient’s needs are met.
• Report directly to the Medical Director and collaborate with operational leadership to implement best practices and drive improvement initiatives.

Patient Population / Client Focus

The primary focus of this role is to provide care to older adults on Medicare. Our unique approach aims to keep patients healthy and engaged in their communities, enabling them to lead fulfilling lives. By fostering trust and understanding through long-term relationships, our Nurse Practitioners can contribute to improved health outcomes and a better quality of life for our patients.

Qualifications

• A Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or graduation from an accredited Nurse Practitioner program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through the AANP or ANCC.
• An active, non-probationary state Nurse Practitioner license, as well as an active DEA license for prescribing.
• Demonstrated experience in primary care settings, such as internal medicine, geriatrics, or family medicine.
• Familiarity with HCC documentation, ICD-10 coding, and health risk assessments is essential.
• A passion for teamwork and the collaborative spirit to achieve the best results for our patients.
• Bilingual proficiency is preferred to enhance communication with diverse patient populations.

Preferred Experience

• Prior experience in a patient-centered medical home model is advantageous.
• Experience utilizing Electronic Health Records (EHR) systems to document patient information and track care plans.
• A proven track record of engaging with diverse patient populations and addressing their unique health needs.

Compensation and Benefits

Our Nurse Practitioner position offers a competitive annual salary ranging from USD 120,000 to 160,000, based on experience, qualifications, and expertise. In addition, we provide a robust benefits package designed to support our staff:
