Key Responsibilities\n

• Collaborate with primary care providers to identify, treat, and manage a range of mental health and psychosocial issues.
\n
• Conduct thorough patient intakes focusing on diagnostic and functional evaluations, developing actionable treatment goals in conjunction with primary care providers.
\n
• Deliver targeted, brief interventions for patients requiring mental health services, identifying the most effective methods of treatment.
\n
• Evaluate which patients are best served within the primary care setting and which require referrals to specialized mental health facilities.
\n
• Implement follow-up assessments to monitor patient progress and adjust treatment plans as needed.
\n
• Develop personalized relapse prevention plans and support patients in maintaining stability in their mental health.
\n
• Actively detect at-risk patients and create strategic plans aimed at preventing exacerbation of their conditions.
\n
• Coordinate and monitor health service delivery, linking with treatment providers both within and outside of our organization, always with patient consent.
\n
• Maintain detailed documentation of patient progress and treatment plans in accordance with established regulations.
\n
• Educate and empower patients with self-management skills to foster independence in their treatment and recovery processes.

Qualifications\n

• Master's Degree in Clinical Counseling or a related field.

• Current LPC (Licensed Professional Counselor) license is required.

• Strong understanding of developmental psychopathology and DSM-IV diagnoses.

• Effective communication and interpersonal skills to establish rapport with patients and collaborate with interdisciplinary teams.

• Ability to maintain clinical records and prepare comprehensive reports for evaluation and continuity of care.

• Capacity to work independently and exercise professional judgment in a dynamic environment.

Preferred Experience\n

• Previous experience in a clinical outpatient setting is highly desirable.

• Bi-lingual proficiency in English and Spanish is preferred to better serve our diverse population.

• Familiarity with psychodiagnostics, psychotherapeutic techniques, and community resources to effectively assist clients.

Compensation and Benefits\n

The salary range for this position is competitive, ranging from USD 87,000 to 105,000 yearly, commensurate with experience.
