Key Responsibilities\n

• Conduct thorough intakes focusing on diagnostic and functional evaluations, offering tailored recommendations to primary care providers regarding treatment goals.

• Engage in brief, focused therapeutic interventions, responding to the immediate mental health needs of patients.

• Collaborate with primary care providers to identify patients who require in-depth specialized mental health services versus those who can be effectively supported within a primary care setting.

• Initiate follow-ups to monitor patient progress and adapt treatment strategies when necessary.
\n
• Develop relapse prevention plans for patients at risk and aid them in maintaining stable functioning.
\n
• Detect ‘at risk’ patients and collaborate on care plans aimed at preventing the deterioration of their mental health.
\n
• Oversee the coordination of health services related to behavioral health care, fostering links with external treatment providers when appropriate.
\n
• Maintain accurate and timely clinical documentation, detailing patient progress and diagnoses in treatment charts.
\n
• Partner effectively with primary care professionals, ensuring they are well-informed about patient needs and cooperative in the development of comprehensive treatment plans.

• Empower patients with self-management skills and education, fostering their active participation in treatment and recovery.
\n
• Provide consultations to clinic management and team members on behavioral health services, contributing to program evaluation and improvements.

• Support patients in adhering to medical protocols initiated by primary care providers, including strategies for managing potential side effects of treatments.

• Uphold the Code of Ethical Conduct for Clinical Professionals and engage in continuous professional development.

Qualifications\n

• A minimum of a Master’s Degree in Clinical Counseling or a related field.

• Current and valid LPC (Licensed Professional Counselor) License.

• Ability to work effectively within an interdisciplinary team, demonstrating strong communication skills.

• Competency in maintaining clinical records and preparing comprehensive reports.

Preferred Experience\n

• Bilingual proficiency in English and Spanish is preferred but not a requirement.

• Demonstrated experience in telehealth practices or familiarity with virtual counseling tools is advantageous.

Compensation and Benefits\n
