Key Responsibilities\n

Qualifications\n

• A Master’s or doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.

• Current and valid licensure as an LCSW, LCPC, or LMFT in the state of Illinois.

• Demonstrated proficiency in addressing the mental health needs of individuals with medical complexities.

• Prior experience in delivering integrated care and collaborating effectively with medical teams.
