We CARE for our patients like they are our own FAMILY. **Company:** Redcrest Hospice**Schedule:** Monday - Friday | 8:00 AM-4:30 PM | On-call rotation only as back-up**Location:** Lafayette, Indiana and surrounding counties**SUMMARY**A Registered Nurse (RN) I - Hospice provides and plans professional nursing care to patients under the direction of a physician. This role involves delivering direct patient care, assisting with examinations, accurately documenting patient conditions, vital signs, diagnoses, and prescribed treatments. The Level 1 RN also administers treatments and medications as directed, contributing to the comfort and symptom control of hospice patients.**KEY RESPONSIBILITIES*** Provides skilled nursing care in a patient’s home as ordered by the attending physician* Observes and monitors patient conditions* Performs assessments to develop an individualized plan of care and adjusts as needs change* Administers medication as prescribed by the physician* Manages multi-disciplinary care as applicable* Promotes continuity of care with appropriate admissions, transfers and discharges* Counsels’ patient and family on the disease/injury processes and how to manage* Incorporates patient and family in development of plan of care* Listens to patient and family members* Oversees and supervises total care of patient provided by Aides and LPNs* Evaluates the treatments and medical condition of the patient according to the plan of care* Other duties as assigned**QUALIFICATIONS*** Performs work under minimal supervision.
