Responsibilities
- Perform skilled nursing visits, assessments, admissions, recertifications, resumptions of care, and discharges in patients' homes.
- Develop, implement, and update individualized Plans of Care.
- Complete OASIS assessments and clinical documentation accurately and on time.
- Monitor patient progress and communicate changes with physicians and the interdisciplinary team.
- Educate patients and caregivers on disease management, medications, treatments, and safety.
- Coordinate care with physicians, therapists, home health aides, and other healthcare professionals.
- Supervise Home Health Aides in accordance with state and federal regulations.
- Promote safe, compassionate, high-quality patient care while helping patients remain safely at home.
Qualifications
- Graduate of an accredited Registered Nursing (RN) program.
- Current, unrestricted Ohio Registered Nurse license.
- Current CPR certification.
- Minimum of one year of nursing experience required.
- Home Health experience is preferred but not required.
- OASIS experience is a plus.
- Valid driver's license, reliable transportation, and auto insurance.
