Nurse Practitioner / Physician Assistant - Primary Care

Position Overview Join our growing practice as a Nurse Practitioner (NP) or Physician Assistant (PA) and bring exceptional team-based care directly to patients residing at Assisted Living and Memory Care communities in and around Woodbury, MN. Responsibilities Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities. Full‑time providers care for patients Monday–Thursday; option for Fridays to be a clinical or administrative day. Manage a panel of approximately 200 patients (12–16 patients per day). Design and manage your schedule. Develop and implement individualized care plans appropriate to patients and their desires. Receive clinical support from Clinical Assistants and care coordination teams including LPNs, RNs, and social workers. Access diagnostics such as labs and X‑rays as needed. Collaborate with community partners within home‑care, hospice, and other resources as appropriate. Qualifications Graduate of an accredited Nurse Practitioner or Physician Assistant education program. Board certified or board‑prepared through AANP, ANCC, or NCCPA. Licensed or able to obtain licensure through the state Board of Nursing or Board of Medical Practice. Ability to obtain and/or maintain DEA certification for the required state of work. Strong verbal communication skills and excellent interpersonal and customer‑service skills. Valid, unrestricted driver’s license and minimum state‑required liability auto insurance; local travel is required. Preferred Qualifications Minimum 2 years of clinical experience as a Nurse Practitioner or Physician Assistant. Compensation and Benefits Base salary of $125,000 to $170,000 USD, plus quality/productivity incentives. Extensive CME support. Unlimited paid time off and eight observed holidays.
