What You'll Do:
- Provide 1:1 therapy sessions in patients' homes.
- Treat a primarily geriatric population.
- Implement individualized care plans.
- Maintain clear and timely documentation.
- Collaborate with a supportive clinical team.

Qualifications:
- Licensed PTA in the state of NY.
- Degree in Physical Therapy.
- Strong communication and clinical skills.
- Home health experience (preferred, not required).
