To provide social work supervision to staff social workers and to provide necessary back-up when these social workers are unavailable. To participate with Administration and other disciplines in the development of programs, policy, and priority setting regarding Social Services and resident care. To provide direct and indirect service, to identify and meet the medical and non-medical related social and emotional needs of the resident and family at the time of admission, during treatment and care in the facility, and at the time of discharge. Coordinates benefits management, financial and psychosocial planning, discharge planning and external resource referrals. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.Responsible for the overall delivery of services through the Social Service Department.Organizes Social Service programs and monitors the delivery, quality and outcome of services.Develops departmental resident care policies.Functions as the Discharge Planning Coordinator for the facility.Remains current with State and Federal Long Term Care regulations.Attends scheduled meetings with the Administrator and weekly Department Head meetings.Assumes administrative role in special projects as requested by the Administrator.Abides by the Standards of Conduct which is the basis of the Organizational Integrity Program.Compliance with Health Information Privacy (HIPPA) related laws and regulations.Assist resident/family with signatures for the admissions process.Assures families are notified regarding payment status changes.Sends certified letters for non-coverage to families.Assists families in understanding Advanced Directives.Conducts tours of the facility to prospective clients.Collaborates with nursing to determine appropriateness of and obtain resident/family permission for bed transfers.Assists families in understanding payment sources.In collaboration with care coordinator, assists families in understanding all items on admission checklist and obtaining signatures as necessary.Provides supervision to staff social workers.Participates with Administration and other disciplines in the development of programs and policy and priority setting regarding Social Services, and resident care in general. ESSENTIAL SKILLS & EXPERIENCEBachelors or masters degree in social services or related field Two to four years of experience in working with geriatric population, preferably in long term care. Must be Licensed Social Worker (LSW) in the Commonwealth of Massachusetts or Licensed Independent Clinical Social Worker (LICSW) preferred.Bachelors or masters degree in social services or related field; two to four years of experience in working with geriatric population, preferably in long term care. Must be Licensed Social Worker (LSW) in the Commonwealth of Massachusetts or Licensed Independent Clinical Social Worker (LICSW) preferred.
