PER VISIT PHYSICAL THERAPIST HHA

City/State: Bronx, New York Work Shift: Day (8:30 a.m.–5:00 p.m., varying days) Hourly Rate: $43.85 (subject to shift differential and experience) Responsibilities Conduct comprehensive assessments of clients of all ages (infant through adult), covering physical, emotional, and mental status. Develop plans of care with realistic outcomes based on assessment data and client preferences. Perform designated discipline interventions according to agency policies. Provide client and significant other education on prevention or management of illness, functional disability, changes in health status, and on medications and treatments. Evaluate results of clinical interventions to measure progress toward goals. Report all pertinent information about client status and progress to appropriate multidisciplinary health team members. Plan sequences of home visits with attention to acuity, geographical location, and other pertinent considerations. Requirements Graduate of a Physical Therapy program approved by the American Physical Therapy Association. Current New York State licensure. Two (2) years at senior level in the field or an acute medical hospital. Two (2) years of supervised clinical experience in a medical setting preferred, or equivalent combination of education and experience. Valid driver’s license and ability to provide own transportation during working hours for home visits.
