RN - Home Health Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience depending on the specialty. License: Must have license in hand. Certifications: CPR, BLS Must-Have: Must have recent specialty experience. Must be proficient in OASIS and have used it within the last 6 months, or be required to take a retraining course during compliance. All professionals must be proficient in HomeCare HomeBase and must have used it within the last 6 months; if not, a retraining course must be completed during compliance. Description:Provides, coordinates, and directs the provision of home nursing care according to physician orders based on agency policies and procedures through the competent application of the nursing process. Responsible to Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Home visits, periodic office meetings are required. Must have reliable transportation, current driver’s license, and agency-required liability insurance. Average number of patients per day is 5-8. Contractor will be assigned between 5-10 patients per day that equate to roughly 6-8 points. Types of cases include Start of Care, Wound Care, and follow-up visits. Weekend requirements vary per location and profession; please discuss during the interview. On-call requirements vary per location and profession; please discuss during the interview. Must be willing to travel up to 75 miles from home clinic. Mileage will be reimbursed at the IRS standard rate. Travel is compensated from patient home to patient home. The commute to and from the first and last patient is not billable. Locals are accepted at the same rate. Must be open to on-call rotations. All documentation must be completed within 24 hours of the appointment with the patient. Delayed and incomplete documentation impacting billing and follow-up patient care may result in a cancel. Expectations are that the clinician will accept every patient on their schedule regardless of how far they are from their previous appointment. Declining patients may result in a cancel. There are no guaranteed hours with this contract. Clinician must hit weekly productivity points of 30 which equates to 40 hours per week. If not receiving enough patients each week to support their financial needs, there will be an opportunity to float to another nearby clinic to pick up patients. #J-18808-Ljbffr
