New CNP Opportunity at Graham School-Based Health Center/Urbana Community Health Center, Champaign County, OH!Community Health & Wellness Partners is an integrated primary care community health center that utilizes a team-based care model, sharing patient-care responsibilities among members of a team. In addition to providing relevant direct and appropriate clinical care, treatment and diagnosis to patients within the scope of practice for a primary care nurse practitioner, the nurse practitioner works in tandem with the team’s physician to function as a Team Leader in overseeing and directing the full operations of a team-care model, providing whole-person, quality care. Function as a fully accountable colleague with physicians and clinics through collaborative care agreements, and strives to support CHWP’s quality initiatives and requirements necessary and relevant to a Federally Qualified Health Center, and various certification programs, i.e. HRSA, PCMH, Joint Commission, etc.Primary ResponsibilitiesAchieve ResultsEffect improvements in patient well-being through accurate and timely diagnosis and delivery of relevant, high quality medical care.
