Work Environment\n

This position allows for a full-time commitment, working Monday to Friday from 8:00 AM to 4:30 PM, with the potential for occasional evening or weekend hours based on client needs. The role is primarily remote, requiring flexibility to conduct home visits and facilitate continuous care. On-call availability may also be necessary to address urgent situations, ensuring that our clients receive timely support and intervention.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors' psychosocial needs, evaluating their emotional, mental, and social circumstances.

• Collaborate closely with clients and their families to devise personalized care plans that reflect their individual goals and aspirations.

• Provide counseling and emotional support, assisting clients in successfully navigating life's transitions, such as health changes or loss of loved ones.

• Advocate for clients' rights while aiming to improve their access to community resources and services.
\n
• Engage with interdisciplinary teams—including nurses and caregivers—to coordinate care efforts and optimize health outcomes.

• Perform regular client visits to monitor their well-being and address safety and living condition concerns.

• Facilitate open communication and offer conflict resolution assistance among family members involved in a client’s care.

• Educate clients and families about available community programs, resources, and support structures to enhance social engagement and improve the quality of life.

• Maintain accurate records of client assessments, care plans, progress notes, and interactions, adhering to compliance and ethical standards.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

The annual salary for this role ranges from $60,000 to $100,000, reflecting your experience and expertise.
