Schedule: Full-Time, Monday to Friday, 8:00 AM 5:00 PM, 1 weekend per month, 1 on-call shift per week

Compensation: $85,000 $130,000 annually plus $6,000 in first-year bonuses

What You'll Do

Manage a home health caseload across homes and community settings.

Complete OASIS assessments and individualized care plans.

Provide skilled nursing care, education, and coordination.

Collaborate with physicians, therapists, and interdisciplinary teams.

Support patients in maintaining independence and safety at home.

Maintain accurate, timely documentation.

What We're Looking For

Active Minnesota Registered Nurse (RN) license.

3+ years of RN experience, home health preferred.

Strong clinical judgment and independence.

Organized, self-directed, and comfortable in a field role.

Reliable transportation.
