Key Responsibilities\n

• Provide compassionate counseling and support to patients and their families to address adjustment issues associated with detoxification, addiction, and emotional well-being.

• Collaborate closely with the rehabilitation team to devise and implement individualized treatment plans that holistically address each patient's psychosocial needs.

• Assist patients in accessing vital community resources such as support groups, vocational rehabilitation services, and housing assistance to help ease their transition to a stable lifestyle.

• Proactively educate patients and families about available community resources as well as potential post-rehabilitation services to encourage ongoing support.

• Take an active role in discharge planning to ensure smooth transitions for patients returning home or moving to other care settings.
