Job Description

The LVN will report to the Director of Nursing (DON) and work collaboratively with physicians, nursing staff, residents, families, and other departments to provide optimal individualized nursing care for residents in skilled nursing facilities.

• Provide patient and family education tailored to specific needs.
• Document assessment findings, nursing interventions, and patient responses effectively.
• Respond to emergencies and initiate resuscitation measures per protocols.
• Maintain patient confidentiality and deliver non-discriminatory care.
• Assist in the orientation of new staff and in educational activities as necessary.
