Help Me Grow Care CoordinatorStatus: Full-Time, Non-Exempt Hourly Range: $31.00 – $34.00 (DOE) Funding Notice: This position is funded through a grant currently awarded through June 30, 2027. Continued employment beyond that date is contingent upon renewal of grant funding.Reports to: Director of Finance and OperationsPosition OverviewThe Help Me Grow (HMG) Care Coordinator is a full-time, non-exempt position responsible for providing care coordination and navigation support to families with children prenatal through age five. The Care Coordinator conducts phone-based intake, developmental screening (including Ages & Stages Questionnaires), referrals, and follow-up to help families access developmental, parenting, and community resources that support healthy child development.The position works closely with 211 call center staff, First 5 Help Me Grow partners, and community service providers to ensure seamless referral pathways and effective follow-up. Through consistent communication, documentation, and coordination, the Care Coordinator helps families connect with appropriate services and supports early identification of developmental and behavioral concerns.The role is operational and coordination-focused and operates within established program frameworks and procedures. It does not include supervisory or fiscal authority.About Help Me GrowHelp Me Grow (HMG) is a national model that helps communities identify young children with developmental or behavioral concerns early and connect families to appropriate services and supports. In Contra Costa County, Help Me Grow works in partnership with First 5, the 211 call center, and community agencies to provide developmental screening, resource navigation, and coordinated referrals for families with children ages birth through five.Through early identification and coordinated support, Help Me Grow strengthens connections between families, healthcare providers, early childhood programs, and community resources that support healthy child development.Core Responsibilities (Care Coordination)Provide intake, screening, referral, follow-up, and advocacy for families with young childrenServe as a liaison between Help Me Grow services and 211 call center staff to ensure coordinated screening, referral, and follow-upAdminister developmental and behavioral screenings, including Ages & Stages Questionnaires (ASQ)Assist families and providers in understanding and navigating early intervention and child-serving systemsSupport families through information, education, advocacy, and follow-up to address service access barriersEnsure confidentiality and compliance with all applicable privacy and data-protection requirementsProgram Operations, Early Identification & DataSupport and monitor the day-to-day operations of Help Me Grow early identification, screening, surveillance, and referral workflowsAccurately enter, maintain, and monitor client data for tracking, reporting, and outcomesEnsure timely, complete, and accurate documentation and reportingSupport preparation of required program, funder, and internal reportsMonitor relevant best practices, early identification efforts, and system developments at the local, state, and national level and communicate observations to program leadershipParticipate in service quality assurance and continuous improvement activities to enhance effectiveness and cultural relevancy of servicesProgram Support ActivitiesServe as the primary point of continuity for Help Me Grow outreach and partner engagementIdentify, outreach to, and maintain collaborative relationships with child-serving agencies, providers, and community networksCoordinate, track, and support outreach activities, presentations, and community events to promote Help Me Grow and child developmentCollaborate with the Resource Database Manager to ensure timely, accurate participation of partner agencies in the development and maintenance of the resource databaseGather partner and community feedback, identify operational gaps or barriers affecting families, and communicate findings to program leadershipSupport the development and distribution of outreach and educational materialsCoordinate and regularly deliver public speaking engagements and presentations regarding the Help Me Grow system and Crisis Center services, and support participation by other Care Coordinators as appropriateSupport early childhood–related trainings and in-services for call specialists and volunteers, as assignedParticipate in Help Me Grow–related meetings, workgroups, and convenings, as funding allowsThe HMG Care Coordinator may participate in outreach, presentations, and community events.Collaboration & Organizational ParticipationWork closely and cooperatively with program managers, coordinators, call center staff, and community partnersParticipate in staff meetings, trainings, and team activitiesActively contribute to a healthy, respectful, and supportive workplace environmentRegularly drive and/or travel throughout Contra Costa and neighboring counties to perform job duties;
