Key Responsibilities Coordinate and manage home care cases and caregiver scheduling Serve as the primary point of contact for patients, families, and caregivers Handle high-volume calls, follow‑ups, and patient inquiries Ensure accurate documentation and compliance in HHA Exchange Collaborate with nurses, case managers, and clinical teams to support Patients needs Monitor attendance, coverage, and missed visits Maintain daily, weekly, and real‑time updates on case status Provide excellent customer service in a fast‑paced healthcare environment Qualifications 2+ year experience in home care, healthcare coordination, patient services, or medical office setting preferred Strong knowledge of home care operations, scheduling, or case coordination Experience with HHA Exchange, EVV systems, or healthcare EMR platforms is a plus Excellent communication, multitasking, and problem‑solving skillsAbility to manage high‑volume workload and prioritize effectively Bilingual (Spanish/English) preferred Why Work With Us?
