Key Responsibilities\n

• Conduct thorough biopsychosocial assessments tailored to the age and cultural background of clients, addressing mental health, substance abuse, and medical needs within the primary care setting.

• Provide evidence-based behavioral health services, including individual and group counseling, intake evaluations, treatment planning, crisis intervention, and necessary referrals to substance use disorder treatment.
\n
• Document evaluations, progress notes, and counseling sessions accurately in our electronic health record system to ensure compliance with legal and organizational standards.
\n
• Serve as a liaison between patients and community resources, facilitating access to medication assistance programs, housing support, and other relevant services.
\n
• Collaborate with Quality Improvement teams to design and implement data collection processes that inform clinical practices and enhance service delivery.

Qualifications\n

• Must hold an active state license as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
