Key Responsibilities\n

• Conducting thorough assessments of clients to identify counseling needs and create personalized treatment plans.

• Utilizing DSM criteria to diagnose substance use disorders, and determining appropriate levels of care based on ASAM criteria.

• Developing, updating, and implementing the counseling portion of client treatment plans, ensuring they align with best practices in substance use treatment.
\n
• Designing and managing comprehensive discharge and continuum of care plans to facilitate clients' transitions into appropriate post-treatment services.
\n
• Maintaining meticulous documentation of all counseling sessions, assessments, and follow-ups in compliance with clinical standards and regulations.
\n
• Managing a caseload of clients, including regular case management and utilization review updates, to ensure effective treatment delivery.
\n
• Coordinating support services for clients and their families, including documenting service provisions and outcomes in clinical records.
\n
• Engaging with external referral sources such as legal, educational, and employment representatives to promote a holistic approach to client care.
\n
• Facilitating interventions for crisis-oriented issues in collaboration with a multi-disciplinary team.

The LCSW will work primarily with clients who are grappling with substance use disorders alongside mental health concerns. Qualifications\n
