Key Responsibilities\n

As a Licensed Clinical Social Worker at CHDC, you will be responsible for a variety of essential tasks:

• Respond promptly to psychiatric crises, ensuring immediate communication with the Medical Director and Chief Medical Officer as required.

• Maintain comprehensive and timely documentation in patient clinical records, adhered to legal and organizational standards.

• Participate actively in Quality Improvement initiatives, contributing to ongoing learning and development within the organization.

• Utilize evidence-based screening tools to assess patients' needs, enabling effective monitoring and reporting of their health conditions.

• Collaborate with the Center of Excellence Team to provide specialized support for patients dealing with Opioid Use Disorder.

Patient Population / Client Focus\n

This role primarily focuses on patients facing a range of behavioral health challenges, including those with chronic illnesses and complex needs. You will work closely with individuals requiring mental health support, emphasizing integrated care approaches that address both physical and psychological well-being. This population may include individuals suffering from substance use disorders or those receiving end-of-life care at our Hospice Center.

Qualifications\n

To be considered for this role, candidates must possess:
