Job Description
Exciting Opportunity for LPNs at LTAC!


- Start Date: CRU MUST START IN THE NEXT WEEK
- Background Check: 7-year background check + FACIS 3 + NY State check on file at time of submission
- Requirements:
- AHA BLS certification (attach any others)
- LTC experience
- Minimum of 1 year of experience
- Flexibility to cover CNA duties as needed
- Schedule:
- 6-week assignment
- Minimum of every other weekend
- Schedule adjusted to meet unit and patient care needs
- Monday to Sunday availability
- No call duty, no self-scheduling, but may pick up shifts
- Travel Information:
- Rates are tiered based on mileage (direct line measurement)
- Local (50 miles or less)
- Radius (51-100 miles)
- Travel (101 miles+)
- Additional Information:
- No block schedules for any reason
- Employment gaps over 90 days must be explained
- Complete work history needed (bed count, EMR, facility type, etc.)
- Public transportation is NOT accessible in this area; reliable transportation is a must (personal or rental vehicle recommended)
