This position offers the flexibility of remote work while serving patients at our Hospice Center in Crossville, Tennessee. In this remote role, you will collaborate with a multidisciplinary team that includes physicians, psychiatrists, psychologists, and case managers. • Conduct thorough psychosocial assessments to understand the needs and circumstances of patients, developing tailored treatment plans that resonate with individual family dynamics.

• Provide therapeutic services, including individual, group, and family therapy, utilizing evidence-based practices such as Cognitive Behavioral Therapy (CBT), Dialectical Behavioral Therapy (DBT), and trauma-informed care.

• Manage crisis intervention situations effectively, offering immediate support and connecting individuals and families to essential community resources when needed.

• Maintain meticulous and confidential documentation of patient interactions, assessments, treatment plans, and progress in compliance with state regulations and organizational policies.

• Contribute to team synergy by actively participating in case reviews, staff meetings, and interdisciplinary collaboration to ensure holistic patient care.

• Engage in continuous professional development to stay current with best practices in clinical therapy and intervention strategies.

The LCSW will work primarily with a diverse patient population requiring hospice and palliative care, focusing on their unique emotional and psychological needs during a challenging time in their lives. By employing empathy and therapeutic skills, the LCSW will facilitate open conversations about grief, loss, and coping strategies tailored to each patient's needs.

• Active Licensed Clinical Social Worker (LCSW) license in Tennessee, in good standing.

• A Bachelor’s degree in Social Work from an accredited institution is required.

• A minimum of 2 years of clinical experience in mental health or outpatient settings with a focus on therapy delivery is mandatory.

• Expertise in evidence-based treatment methodologies, including knowledge of modern therapeutic techniques.

• Exceptional communication, organizational, and interpersonal skills conducive to patient interaction and team collaboration.

• Familiarity with electronic medical record (EMR) systems will be advantageous.
