MSW Case Manager - Mental Health & Discharge Planning

Sierra Vista Hospital

The Sierra Vista Hospital location, a 171-bed psychiatric care facility located in south Sacramento, is a full-service mental health facility and a center of support for patients and their families. The Case Manager is responsible for managing the treatment activities offered to the patients, which may include facilitating therapy groups on patient units; complete thorough assessments; assessing patients' needs and compiling a treatment plan within a multidisciplinary team; planning and/or #J-18808-Ljbffr Sierra Vista Hospital
