Case Manager II: Behavioral Health & Discharge Planning

Universal Health Services

Sierra Vista Hospital in Sacramento seeks a full-time Case Manager II in the Social Services Department. You will assess patients, coordinate discharge planning, and work with a multidisciplinary team to develop care plans for individuals facing behavioral health challenges. You will collaborate with treatment providers and external partners, ensure follow-up resources, and mentor junior staff under the supervision of a Masters level clinician. #J-18808-Ljbffr Universal Health Services
