Key Responsibilities\n

• Conduct comprehensive intake assessments for newly admitted patients to gauge their mental health needs.

• Develop individualized, client-centered treatment plans in collaboration with patients and their families.

• Deliver evidence-based therapeutic interventions in both individual and family therapy sessions, tailored to the unique needs of patients facing end-of-life challenges.

Patient Population / Client Focus\n

This position focuses on working with a diverse patient population within a hospice setting, encompassing individuals from various backgrounds dealing with complex emotional and psychological challenges related to terminal illness. You will provide support not only to patients but also to their families, addressing grief, loss, and adjustment issues.

Qualifications\n

• A Master's degree in Social Work or a related field is required.

• Active Licensed Clinical Social Worker (LCSW) license in the state of California.

• A minimum of two years of experience in a clinical setting, preferably within hospice or palliative care.

• Strong capability to formulate treatment plans and maintain accurate documentation aligned with clinical best practices.

• Demonstrated cultural competency and skills to effectively engage with a diverse patient population.

Preferred Experience\n

Compensation and Benefits\n

The annual compensation for this role ranges from USD 115,000 to 120,000, commensurate with experience and qualifications.
