Job Summary The Social Worker, under direct and continuous supervision, provides comprehensive biopsychosocial/spiritual assessment, diagnosis, and treatment/interventions/advocacy of patients and their support systems. The licensed clinical social worker independently provides similar services and collaborates with the inter‑/multi‑disciplinary team to develop evidence‑informed, person‑centered care plans. Responsibilities Collaborate with patients and their support systems to create and implement care plans; conduct biopsychosocial assessments; provide case management; advocate for patient self‑determination; work across inpatient and outpatient settings; adhere to NASW code of ethics and Standards of Practice; reduce health disparities and barriers to care. Social Worker MSW Qualifications Master’s degree in Social Work. Oregon Clinical Social Work Associate license within 60 days of hire. Social Worker LCSW Qualifications Master’s degree in Social Work from a CSWE‑accredited school, or a school in candidacy status. Oregon or Washington Clinical Social Worker license upon hire (or license in any state where care is delivered). At least 2 years of relevant social work experience; may include CSWA experience. Preferred Qualifications Experience in clinical work within healthcare, hospice, or bereavement settings. Experience in acute care or care management settings.
