Experience with frail/elderly populations or willingness to complete training
Strong clinical and independent decision-making skills
Clean malpractice/license history
Most recent NPDB Self Query report (within 30 days active)


Description:
Join a collaborative interdisciplinary team delivering high-quality, coordinated primary care to geriatric patients.
Provide primary medical care in collaboration with physicians.
Perform initial, semi-annual, annual, and as-needed participant assessments.
Develop and integrate individualized care plans within the interdisciplinary team framework.
Evaluate and treat acute illnesses and manage chronic conditions.
Deliver preventive care including screenings, immunizations, and monitoring.
Prescribe medications and therapies appropriately.
Refer participants to specialists when needed.
Participate in discharge planning and transitions of care.
Educate patients, families, and caregivers on treatment plans and health management.
Serve as an active member of the interdisciplinary team.
Assist with the development of clinical policies, procedures, and care standards.
Participate in staff education and training initiatives.
Maintain accurate and compliant clinical documentation.
Participate in on-call rotation.
Ensure compliance with all regulatory and organizational policies.
