Job Overview Specialty : Geriatric Medicine Hospital Location : Kalispell, MT Setting : Skilled Nursing Facility (SNF) and Long-Term Care (LTC) unit Board-certified or board-eligible : in Internal Medicine or Family Medicine physician with experience in Skilled Nursing, Long-Term Care, Transitional Care, and post-acute medicine Dates needed : Sept 15-17, Sept 29 - Oct 1, Oct 13-15, Nov 3-5, 10-12, 17-19, 23-25 Hours : 7a-5p Patients for Shift : 12-14 License : MT license or IMLC Call : No, Hospitalists cover night call EMR : MediTech Responsibilities include : Routine medical evaluations and chronic disease management. Annual wellness and preventive care assessments. Management of dementia, frailty, and other complex chronic conditions is common in the long-term care population.
