Family Practice - Geriatrics Physician

Opportunity Highlights Schedule: Three days per week, preferably Tuesday, Wednesday, and Thursday; 10-hour shifts, 7:00 AM - 5:00 PM. No call. Hospitalists cover night call. Specific Dates Needed: September 15-17 September 29 - October 1 October 13-15 November 3-5 November 10-12 November 17-19 November 23-25 (Monday-Wednesday due to holiday) Job Setting: Outpatient and Inpatient Types of Cases: Geriatric and medically complex patients including short-term rehabilitation, long-term care residents, chronic disease management, dementia, frailty, palliative and comfort-focused care, post-acute medicine, and transitional care Patient Volume: 12-14 patients per shift Call: No call EMR System: MediTech Credentialing: Estimated 120 days; temporary/emergency privileges available Primary Responsibilities Review and approve admissions to the Skilled Nursing Facility, including completing comprehensive history and physical examinations for all new admissions Conduct required physician compliance and recertification visits in accordance with CMS and regulatory requirements for both SNF and LTC residents Provide ongoing primary medical care for long-term care residents, including the management of chronic medical conditions and preventative care Develop, review, and update individualized care plans in collaboration with nursing, rehabilitation services, case management, and social work Manage acute changes in condition, evaluate new symptoms, and provide treatment to reduce unnecessary emergency department transfers and hospital readmissions Prescribe, monitor, and adjust medications while reviewing consultant pharmacist recommendations and ensuring medication safety Participate in interdisciplinary care conferences, family meetings, and goals-of-care discussions as appropriate Coordinate transitions of care between the hospital, skilled nursing, long-term care, specialty services, and community providers Long-Term Care Responsibilities Routine medical evaluations and chronic disease management Annual wellness and preventive care assessments Management of dementia, frailty, and other complex chronic conditions common in the long-term care population Oversight of palliative and comfort-focused care plans when appropriate Collaboration with families and healthcare decision-makers regarding changes in condition, treatment plans, and advance care planning Certification and documentation required to maintain residents' long-term care plans and regulatory compliance Minimum Requirements Board Certified/Board Eligible: Board certified or board eligible required; Internal Medicine or Family Medicine with experience in Geriatric Medicine, Skilled Nursing, Long-Term Care, Transitional Care, and post-acute medicine Fellowship: Required Certifications: ACLS required; BLS required Licensure: Licensed in Montana (MT), will consider IMLC About VISTA Staffing A ClearlyRated Best of Staffing Client and Talent 10-Year Diamond Award winner, VISTA has 30 years of experience optimizing continuity of care for hospitals, medical practices and government agencies across the US.
