Telephonic Social Work Care Manager - Medicare/Medicaid

MJHS

MJHS is seeking a Social Work Care Manager to support Telephonic member care management across medical, psychosocial, and spiritual needs. You will develop, implement, and monitor person-centered service plans while coordinating with the interdisciplinary team and PCP to ensure continuity of care across settings. The role requires a social work degree with MSW/LMSW preferred and 3–5 years of clinical experience in geriatrics, home care or managed care, along with working knowledge of #J-18808-Ljbffr MJHS
