Nurse Practitioner - Community Health & HIV Care

Care-For-the-Homeless- is seeking a licensed Nurse Practitioner to join the PCMH team at a contracted facility in New York. You will report to the collaborating physician and engage in committee work and peer review as requested. Responsibilities include taking health histories, performing physical exams, and on-site HIV testing; managing acute and chronic illnesses; advising patients on health and mental health; and coordinating referrals when needed in a non-traditional setting for homeless #J-18808-Ljbffr Care-For-the-Homeless-
