Admissions Registered Nurse

Now Hiring - Admissions Nurse Hours: Tuesday-Friday days, noon to 10pm The Admissions Nurse , will be responsible for but not limited to: Completes an initial assessment of patient and family to determine home care needs and identifies problems for each patient upon admission. Ability to outline the patient/family referral and admission. Uses assessment data to determine care plan needs and initiates the plan of care. Articulates hospice philosophy, goals and objectives.
