Work Environment

You will be operating primarily on-site in patients' homes, which requires adaptability and a proactive approach to patient care. Key Responsibilities

• Conduct comprehensive evaluations of patients, assessing their functional limitations and creating tailored treatment plans aimed at achieving rehabilitation goals.
• Implement therapeutic interventions, closely monitoring progress and adjusting treatment methodologies as necessary.
• Perform ongoing assessments and re-evaluations, documenting all findings accurately in compliance with relevant laws and organizational standards.
• Maintain effective communication with patients, their families, and the interdisciplinary care team, providing updates and information regarding treatment plans and patient progress.
• Provide consultation to healthcare providers to ensure a smooth continuum of care.
• Participate in community outreach initiatives and quality improvement projects to enhance our service offerings.
• Supervise and mentor support staff, ensuring that all team members adhere to best practices in patient care.
• Engage in continuous education and professional development to remain at the forefront of physical therapy practices and innovations.

Patient Population / Client Focus

The patient population you will serve includes individuals with varying levels of mobility challenges due to orthopedic conditions, post-surgical recovery, neurological disorders, and age-related ailments. You will develop personalized care plans that account for each patient's unique needs, emphasizing their functional goals to enhance their independence at home.

Qualifications

• Bachelor's Degree in Physical Therapy is required.
• A valid state license in Physical Therapy and BLS certification are mandatory.
• Demonstrated proficiency in verbal and written communication skills.
• Computer literacy, including familiarity with electronic medical records (EMR) systems.

Preferred Experience

The ideal candidate will possess at least two years of relevant experience in a physical therapy role, preferably in home health or rehabilitative settings. Experience working with a diverse patient population is also advantageous.

Compensation and Benefits
