Hospice RN Case Manager — Lead Interdisciplinary Care

aah in Illinois is seeking a full-time Hospice Case Manager to coordinate patient and family care across hospice settings, leading the interdisciplinary team to implement and update the plan of care for terminally ill patients. You will ensure comfort, dignity, and high-quality end-of-life care while navigating complex medical needs and family dynamics. RN licensure and BLS certification are required. Expect 40 hours weekly (Mon–Fri, 8:00 AM–5:00 PM) with on-call for holidays; service areas #J-18808-Ljbffr aah
