AdventHealth is seeking a Field Hospice Nurse to provide nursing services to patients at home or in nursing facilities across Adams, Arapahoe, Jefferson, Denver, Douglas, Elbert and Elizabeth Counties. The role includes care plan initiation, coordination of CNAs, patient assessments, and family education.
