Responsibilities

• Provide direct patient care under the supervision of a supervising clinician, including routine nursing procedures and assessments.
• Administer prescribed medications and monitor for adverse reactions, documenting outcomes accurately.
• Assist residents with activities of daily living and support mobility, feeding, hygiene, and comfort needs.
• Monitor vital signs, manage wound care, and respond to changes in patient conditions per care plans.
• Maintain clear, timely charting and effective communication with the care team, including oral and written handoffs.
• Ensure safety, infection control, and adherence to regulatory requirements within the long-term care setting.

Requirements

• Current Licensed Practical Nurse license with eligibility to practice in New Mexico (NM preferred)
• Willingness to work in a long-term care setting and participate in a 3x12-hour shift pattern
• Local assignment eligibility; able to work within the Albuquerque area
• Strong clinical assessment, documentation, and communication skills
• Ability to comply with care plans, policies, and regulatory requirements
• Demonstrated reliability and professionalism in a high-demand care environment
