Responsibilities

• Deliver direct patient care to residents in a long-term care setting according to individualized care plans.
• Administer medications and treatments per physician orders; document nursing notes accurately.
• Monitor resident status, assess changes, and promptly report concerns to the supervising clinician; respond to call bells and emergencies as needed.
• Collaborate with an interdisciplinary team to implement care plans and ensure continuity of care and safety.
• Maintain thorough, compliant documentation in the electronic medical record and uphold infection control and safety standards.

Requirements

• Active LPN license in good standing in New Mexico
• LPN diploma from an accredited practical nursing program
• Current Basic Life Support (BLS) certification
• Ability to work 12-hour day shifts with a 36-hour weekly schedule
• Prior experience in long-term care or skilled nursing is preferred
• Strong communication, documentation, and teamwork skills
